The Patient Intake Form is in a format called Acrobat (or PDF) that requires a free plug-in program to allow you to open it and print it. We are using this format because it ensures that the form will print out the same and with no problems to you. If you do not have the free Adobe Acrobat Reader program on your computer, then please click here and follow the instructions on the resulting web page to get the Acrobat Reader.

With the Acrobat Reader installed, you may click on the link below to open the Patient form in a separate window. From there you only need to press the "Print" button (or on the file menu click "File", then "Print...") to print out the form.

Once you have printed all 3 pages allow yourself approximately 15 minutes of uninterrupted time to accurately and honestly complete the form. Be sure to bring the form with you on your initial visit! If you haven’t already scheduled your initial visit just call the office at the above number. Our staff will guide you through the rest of the process. It’s that easy!
